Florida has been issuing computer certification since the
mid 1980s, which is accepted by the U.S. Passport Office, the
Social Security Administration, courts, school districts, and all
other state and federal entities and cruise lines requiring proof
of U.S. birth.
Remarks: All requests must include a copy of
State-issued or Government-issued document or Photo ID of the
person requesting the Certificate. Expired cards or documents will
not be accepted.

If you are not an immediate family member, you must provide
legal documentation, such as a court order establishing
guardianship, that documents a direct and tangible interest in the
birth certificate.

Time-frame: State Records dating back to 1865
Notes: Additional copies of the birth record
ordered at the same time are $4.00 each.
Remarks: For births that occurred within the
past 75 years, copies can be requested only by the immediate family
of the person whose name is on the birth certificate.
Computer Certification - Normal processing time is 3-4 business days, provided the record and application are complete and in order.
Remarks: The Computer Certification is recognized and accepted by ALL State and Federal Agencies.
Photocopy Certification - Normal processing time is approximately 10 business days. Definitions of the two types of Certifications are on the reverse side.

Office of Vital Statistics. The Heirloom Birth Certificate makes a
great gift, especially when it's engraved with baby's name and
birth date.

Apostille/Exemplified Certificates
An Apostille is a certification provided under the Hague
convention of 1961 for authenticating documents for use in foreign
countries. The sole function of the Apostille or Exemplified
certificate is to certify the authenticity of the signature of the
document.
Remarks: In Florida, the Apostille or
Exemplified certificate contains the original signature of the
State Registrar and the Secretary of State.
A Claim of Paternity may be filed at any time prior to the
child's birth BUT a claim of paternity may not be filed after the
date a petition is filed for termination of parental
rights.

Certificate of Birth Resulting in Stillbirth
A Florida Certificate of Fetal Death must be on file before
the Certificate of Birth Resulting in Stillbirth Certificate can be
issued. Statute defines a fetal death as one in which the 20th week
of gestation has been reached.
Certificate Fee: $9.00
Additional Copies: $4.00
Additional Years: A fee of $2.00 per year is
required and the maximum search fee is $50.00.
Notes: If a fetal death record is not found, a certified "No Record Found" statement will be issued. Normal processing time is 5-7business days. Issuance is from the State Bureau of Vital Statistics only; county offices cannot issue this certificate, but should refer applicants to the state office.
In addition to having the physician arrange for the
disposition of fetal remains under the terms and conditions
customarily used, you may also choose for burial or cremation by a
licensed funeral director or registered direct
disposer.

Notes: As a result of a spontaneous fetal demise
of less than 20 weeks gestation, this form is provided to advise
you of your options for final disposition of fetal remains.
Remarks: If you choose not to sign this form, the physician is authorized by law to arrange for the disposition of fetal remains under the terms and conditions customarily used.
Funeral Director/Direct Disposer is to file this report with
the Local Registrar of the county in which death occurred each time
a death certificate is not complete upon the termination of the 5
day extension indicated on the Burial-Transit Permit which was
filed for this decedent.

funeral director must provide written justification to the
registrar." Chapter 382.008(3)(b) Florida Statutes.
Remarks: If the physician or medical examiner
has indicated that he or she will sign and complete the medical
certification of cause of death, but will not be available until
after the 5-day registration deadline, the local registrar may
grant an extension of 5 days.

A Certification of Marriage (June 1927 to present) that has
been recorded by the Clerk of Court. This certification is accepted
by all State and Federal Agencies and used as evidence of the
marriage. Normal Response time is 5-7 business days, provided the
record and application are complete and in order.

Notes: Beginning with 1972, the application to
marry section was incorporated to the front of the Marriage record.
Additional copies of the birth record ordered at the same time are
$4.00 each.
Remarks: Marriage Licenses from June 6, 1927 to the present are available at this office. Any Marriage Record prior to June 6, 1927 is obtainable from the county Clerk of the Court where the Marriage License was issued

certificates contain calligraphy style printing, gold state seal,
and are suitable for framing and preserving as family

After a divorce has been recorded, the Clerk of the Court
sends a Report of Dissolution of Marriage to this office. If the
divorce was recently granted, it generally takes up to 60 days to
be received by this office from the Clerk of the
Court.

Time-frame: Records since June 6, 1927
Notes: The divorce decree and other court papers
remain on file at the Clerk's office. Additional copies of the
birth record ordered at the same time are $4.00 each.
Remarks: If the current dissolution of marriage is less than sixty (60) days from the date of this application and you need evidence of the divorce for legal purposes, you may wish to contact the county where the Dissolution of Marriage was granted.
