The Tennessee Office of Vital Records reviews, registers, amends, issues and maintains the original certificates of births, deaths, marriages and divorces that occur in Tennessee in accordance with Tennessee Code Annotated.

Remarks: A copy or an extract of most original records can be purchased from the Tennessee Vital Records State Department of Health or the County Clerk's office of the county where the event occurred.

Three ways to obtain a copy of your certificate:

Mail-In - You must complete, sign, and have the application for certified copy of a certificate notarized, and send it to the proper office;

In Person - Requests for certified copies of certificates made by the person named on the certificate, or a parent, guardian, child or spouse of the named person, are accepted at the Tennessee Office of Vital Records in Nashville;

VitalCheck - Secured online applications to request for vital records, which accepts all major credit card payments, can be made through VitalCheck with an additional charge for their service.

All requests must include a copy of Government or State-issued Photo ID and signature of the person requesting the certificate.

Beginning 1 January 1914, statewide registration of vital statistics began but was not generally complied with until 1927.

Most counties kept birth records beginning in 1908. During the month of July, the clerk of the Board of School Directors was required to collect and report the births and deaths that had occurred in his district during the previous year.

Notes: Additional copies of the same record, requested at the same time, are $5.00 each.

Remarks: Birth records more than 100 years old are maintained by Tennessee State Library and Archives.

Cause of Death is available only to the decedent's parent, child, spouse, or an attorney or agency acting on behalf of the decedent's estate or qualifying family member.

Remarks: If you don't find an expected record, try browsing for the surname only, and with various spellings of the surname. For earlier records write to the Vital Records Office in the county where the event occurred.

County marriage records are the earliest and most complete vital records for Tennessee. The records which have survived usually begin within a few years of each county's organization.

Remarks: Extracted marriage records for many counties to about 1890. Not all years for all counties. Use variant spellings or wildcard search for surnames. Search fee is free.

Tennessee State Library and Archive has a state-wide divorce index which covers the dates July 1, 1945 - December 31, 1960. If the divorce took place during this time period, TSLA will search a five year range state-wide for a divorce.

If the divorce took place prior to July 1, 1945, then TSLA will search a five year range in ONE specified county for a divorce.

Remarks: Court minutes are another location to search for records concerning the granting of a divorce.
