The Division of Vital Records Administration (DVRA) issues certified copies of birth, death, marriage, divorce and civil union certificates to qualified individuals and agencies that provide a "direct and tangible" interest in obtaining a record.
In order to obtain certified copies of Vital Records, you must complete the above application and send it to the DVRA, or visit your local City or Town Clerk, and pay a search fee.
Remarks: Effective January 1, 2005 all
individuals requesting a certified copy of a record (Pursuant to
RSA
5-C:9) must present positive identification, including, but not
limited to, a driver's license, passport or other picture
identification, or in the absence of acceptable picture
identification shall complete the form

New Hampshire Vital Records Information Network web query site, is a service of the New Hampshire Department of State's Division of Vital Records Administration. This site will allow you to query New Hampshire's events for birth, death, marriage and divorce.
Remarks: The Department of State developed NHVRINweb to satisfy the intent of this legislation, and to get Vital Records data in the hands of researchers and the general public.
VitalCheck - Secured online applications to request for vital records, which accepts all major credit card payments, can be made through VitalCheck with an additional charge for their service.
Application to correct or complete a certificate must be
made to the clerk of the city or town in which the event occurred.
The fee for making a correction on a certificate is $10.00. This
fee must accompany the application.
If a certified copy of the record is desired, there is an additional fee of $15.00 per copy. Checks should be made payable to: the City/Town Clerk of that city or town where the event occurred.
Remarks: There is a special form used for the purpose of applying to amend a certificate. This form is called "Application for correcting or completing a Certificate of Birth, Marriage OR Death". Most applications are made on this form.

Heirloom Birth Certificate
The New Hampshire Heirloom Birth Certificate is 11 x 14 inches (suitable for framing) and is created from the original artwork of NH artist Karen Holman and is now being offered at a cost of $25.00 through the New Hampshire Division of Vital Records Administration.
Remarks: The Heirloom Birth Certificate is available to all New Hampshire-born citizens that were born between the years of 1948 to the present. The recipient of the certificate must be able to meet New Hampshire's access requirements.
