The Nebraska Division of Vital Records (Nebraska Vital Records) issues certified copies of Nebraska birth certificates, Nebraska death certificates, Nebraska marriage records, and Nebraska divorce records for events which occurred within the State of Nebraska.
Certified copy means a certificate certified by the Department as provided by law. Certificate means the form used for registration or reporting to the Department the event of birth, death, marriage, or dissolution of marriage as approved by state law or regulation.
Remarks: A valid/current government, school
or employer issued photo identification document of the applicant
is required before a search of the records will be conducted.
Requests that do not contain proper identification will be returned
unprocessed.

To correct a Birth Certificate, one of the
witnesses on the affidavit must be the person whose birth is
registered on the certificate or his/her parent, guardian, or the
medical records clerk of the hospital where the birth
occurred.
To correct a Death Certificate, one of the witnesses on the affidavit must be the funeral director, certifier or informant listed on the certificate.
Notes: A fee of $40.00 includes one copy of the corrected certificate should be sent to the Bureau of Health Planning, Statistics and Emergency Response, Office of Vital Records, 4150 Technology Way, Suite 104, Carson City, Nevada 89706.

A person for whom a delayed certificate of birth is to be
filed must have been born in Nevada, and a birth certificate must
not have been previously filed in this state.
Before filing a delayed certificate of birth the following facts must be established concerning the person whose birth is to be registered. Date of birth, place of birth and parentage.
Notes: The filing fee per N.R.S. 440.700 is $40.00. Additional copies are $20.00 each.
Remarks: You need the enclosed Affidavit to Establish the Facts of Birth completed by someone that witnessed or is knowledgeable of the birth other than the parents, and the Information form so we can create the delayed certificate of birth.
A Certificate of Foreign Born (CFB) is a record of birth
established in the Nevada State Office of Vital Records to replace
the birth record of an individual born in a foreign country to
foreign parents.
The purpose of establishing a CFB is to have a functional birth record in the English language, for use in employment, school, etc.
Notes: The filing fee per N.R.S. 440.303 is $40.00. Additional copies are $20.00 each.
Remarks: The record remains on file in the Nevada State Office of Vital Records, and certified copies may be purchased in the same fashion as Nevada-born records.

There is a $40 charge to amend a certificate already on file in the
office of Vital Records.
This fee includes a certified copy of the amended or new record. Additional copies are $20 each. Please make out your cashier's check or money order to Nevada Vital Records.

The original information about the child to be adopted must
be provided on the first part of the application as it was before
the adoption. Without this data it may be impossible to locate and
amend the child's birth record.
Adopting parents must furnish the following information concerning themselves. The information will be used to prepare a new birth certificate. Do not use initials.
Notes: Please remit $40.00 filing fee. This provides one certified copy of the amended birth record. Additional copies are $20.00 each.

Statewide registration of marriages began in 1968; however, many Nevada counties began keeping records at a much earlier date. To obtain a marriage record, write to the county recorder of the county where the event took place.

Divorce cases were handled by the probate courts until 1864 and by the district courts after that date. The Nevada State Library and Archives has the early probate court records.
To obtain a record of a divorce since 1864, write to the clerk of the district court in the county. The documents are open to the public.
