Applications to request for vital records can be made in person at the State Office of Vital Records in Phoenix. You can also send your request by mail, or file a request for expedited service on-line.
To accommodate a large volume of applicants, disabled & non-disabled free parking spaces are available within the Office parking lot and an additional parking space one block east in our parking garage on 18th Avenue, midway between Van Buren and Adams Streets.
Remarks: All requests must include a copy of
State-issued or Government-issued document or Photo ID of the
person requesting the Certificate. Expired cards or documents will
not be accepted.
Secured online applications to request for vital records,
which accepts all major credit card payments, can be made through
VitalCheck with an additional charge for their
service.

Notes: Some county offices in Arizona are able to
provide certified copies of birth certificates. To verify current
fees, (602) 364-1300 for a recorded schedule of fees.
Remarks: State office has records since 1890's;
however, many events before 1930 were never registered with the
Bureau. Beginning January 1, 2007 all documents support a
correction or an amendment to a birth record must be translated in
English by a certified translator.

Notes: Home birth is defined as: the child and
placenta are born away from a hospital or birthing facility, and
not en route to a hospital or birthing facility.
Remarks: The child's father, mother, other
family member, or a person present during or immediately after the
birth may register the birth if they are at least 18 years. If a
home birth has not been registered more than 1 year after birth,
you must apply for a delayed birth certificate.

Notes: Delayed Birth Certificate or the
Certificate of Late Registration is applied when a child is born
outside the hospital or birth centers, and the parents did not
apply for birth cetificate before the childs' first day.

Notes: Adoption is a process completed by a court
of competent jurisdiction. The Office of Vital Records is
responsible for amending the birth certificate after an adoption
has been finalized. By the tenth day of the month, all adoptions
finalized in the preceding month, shall be forwarded by the court
to the State Office of Vital Records for the birth certificate to
be amended to reflect the adoptive information.

Notes: The Office of Vital Records shall establish
a Foreign-Born Birth Certificate for an adopted person if he's born
in a foreign country and not a United States Citizen at birth, or
he's being adopted by a resident of Arizona or the adoptive parents
or adopted person has completed and submitted the appropriate legal
and evidentiary documents.
Amendment/Correction on a Birth Certificate

Notes: Corrections and amendments to vital records
are governed by Arizona statutes and administrative codes. The
Office of Vital Records must abide by these laws when making
amendments and corrections.
Remarks: The process to make changes to a birth
or death certificate varies depending on the type of legal change
needed and how recently the record was filed. You may want to call
before coming to our office and find out what documentation will be
required.

Notes: Parents may choose to complete the
Acknowledgment of Paternity at the hospital when the child is born
or may do it later. In either case, the father's name will not be
added to the birth certificate until the form is filed by both
parents, with the County or State Office of Vital Records, or the
DES Hospital Paternity Program.

Notes: A putative father is a person who claims to
be the father of a child and wants to establish paternity. This
person also wants to receive notices of any adoption proceedings
concerning the child.
Remarks: The Putative Father Registry was
established by Arizona Revised Statute 8-106.01 and is maintained
by the Arizona Department of Health Services, Office of Vital
Records. According to statute, the Department of Health Services,
Office of Vital Records shall provide the Notice of Claim of
Paternity form and issue the forms to any potential putative
father.

Notes: A fetal death certificate is filed whenever
a fetus dies before birth, and; the fetus is more that 20 weeks in
gestational age or weighs at least 350 grams.
Remarks: In some cases, you may obtain a fetal
death certificate for a fetal death that occurred within the last
30 days at the county health department in the county where the
death occurred. If the fetal death occurred within the last 30
days, click here for a list of county offices. Please note that not
all Arizona counties offer this service.

Notes: Applications are available in the lobby of
the Office of Vital Records and may also be printed from this web
site. Certified copies of a Certificate of Birth Resulting in
Stillbirth can be requested in person at the customer service
counter at the Office of Vital Records or by mail.
Remarks: In 2001, the Arizona Legislature
established a new certificate entitled "Certificate of Birth
Resulting in Stillbirth." This new certificate became available
from the Office of Vital Records beginning August 9, 2001.

Notes: In Arizona, official recording of vital
records was not required until 1909, but many records of marriage
exist from earlier dates in each county.
Remarks: Marriage records are maintained by the
Clerk of the Superior Court in the county where the event occurred.
(This also applies to Arizona divorce records.)

Notes: If you are under the age of 18, you must
either have a notarized parental Clerk's Office consent form or
have your parent(s) accompany you, present the proper
identification, and sign the parental consent form in front of the
clerk issuing your license.
Remarks: A copy of a divorce decree is not
required. No blood test is required. You will receive your marriage
license and can be married on the same day you apply for it. An
unused marriage license expires one year from the date of purchase.
Both parties must be present to obtain a marriage license.

Notes: Couples who are already married may convert
their existing marriage to a covenant marriage. It does not matter
where their license was issued. Solemnization (a wedding ceremony)
is not required to accomplish the conversion to a covenant
marriage.
Remarks: Effective Aug. 21, 1998, couples who
are planning to apply for a Covenant Marriage License must provide
a notarized statement from the clergy or marriage counselor who
provided their premarital counseling.
