The Center for Health Statistics is responsible for issuing certified copies of vital records, including birth, death, marriage and divorce certificates for events that occurred in Oregon. CHS is responsible for registering, certifying, amending, and issuing these Oregon vital records.

Oregon law requires that all vital events such as births, marriages, divorces, and deaths be recorded and registered. These are the legal records documenting a vital event and, in the case of birth certificates, are the primary documents used to establish identity.

Remarks: All individuals requesting a certified copy of a record, must present/submit copies of positive signed identification, including, but not limited to, a driver's license, passport or other picture identification.


Apostille and Authentications

The Oregon Secretary of State can certify to Oregon notaries public and the State Registrar for vital records (Oregon Center for Health Statistics). The procedures are outlined below.

It normally takes two (2) to three (3) days to process a request. If you need an apostille or authentication certificate for a county document, please call our office, (503) 986-2593, before sending it in. Most county records cannot be authenticated.

Remarks: A certified copy of a county record is not appropriate and should not be sent to us. Only the Oregon Center for Health Statistics can issue a true, current vital record.


Amendments & Corrections

The amendment process varies depending on the type of legal change needed, and how recently the record was filed.

Therefore the first step in making a legal change to a Vital Record is to contact the Center for Health Statistics to get detailed instructions in regards to the legal change needed.

Remarks: Fees for amendments vary. In most cases where a hospital, birthing center or funeral home requests a correction within a year of the birth or death, the $30 amendment fee is waived.


Options to obtain a copy of vital record:

Order In Person

Come in person to the Vital Records office, Monday through Friday between 8:00 a.m. and 4:30 p.m. The order counter closes at 4:45 p.m. The average wait time to receive a record is 15-20 minutes.

Expect longer wait times of 30-45 minutes Mondays and Fridays and daily from 11:30 a.m. to 2 p.m. Orders need to be placed by 4:30 p.m. to receive them the same day.

Remarks: The non-refundable fee is $20 and every additional copy of the same record ordered at the same time is $15.

Ordering By Mail

Sending a Request by Mail: you must include an order form, complete vital record information and a photocopy of your driver's license or official ID.

Please allow three to five weeks for processing and delivery. The non-refundable fee for one copy is $20.00 and each additional copy of the same record ordered at the same time is $15.00.

Ordering by Fax

You may send a fax directly to: (503) 234-8417. Please include complete vital record information and identification documentation and include your credit card number with expiration date.

The non-refundable fee is $32.50 for the first record. This fee includes a $12.50 quick service fee. $7.00 of the $12.50 quick processing fee will be charged by VitalCheck and cannot be canceled or refunded once the order is entered.

Order by Phone

Dial (971) 673-1190 Monday through Friday, 8:00 a.m. to 4:30 p.m., PST. Orders must be placed prior to 4:30 p.m. The non-refundable fee is $32.50 for the first record.

This fee includes a $12.50 quick service fee. $7 of the $12.50 quick processing fee will be charged by VitalCheck and cannot be cancelled or refunded once the order is entered.

VitalCheck

The order must include complete vital record information and order processing documentation. Internet orders must be billed to a credit card.

The non-refundable fee for one copy is $32.50 which includes a $12.50 quick processing fee. $7.00 of the quick processing fee will be charged by VitalCheck and cannot be cancelled or refunded.

Remarks: Oregon State Archives has birth records for the City of Portland from 1864 to 1902 and statewide delayed birth records from 1845 to 1902.


Oregon Vital Events Registration System (OVERS)

The Oregon Vital Events Registration System (OVERS) is a multi-year project to modernize Oregon's vital records systems.

The new system provides a secure Web-based environment that supports convenient, stable and flexible operations from the creation of certificates to the production of certified copies and statistical reporting.

Remarks: Self-paced training is on our website 24/7, allowing you to follow and interact with computer-based training content from any computer with an internet connection at anytime of the day or night.


Order Heirloom Certificates

A 9" x 20" heirloom certificate is available with birth data entered with calligraphy and with the signature of the Governor of Oregon and the State Registrar.

If you are not an immediate eligible family member then the heirloom birth certificate must be mailed directly to the child or his/her parents. Please provide the address of the child or his/her parents.

Remarks: Heirloom birth records are certified documents suitable for framing. The price is $45.00, $15.00 of that is tax deductible since it supports the Children's Trust Fund, an agency dedicated to preventing child abuse.


Order Pre-adoption Birth Certificate

Pre-adoption birth records may only be ordered by the adoptee and will only be mailed to the adoptee or their legal representative.

In a few cases where the sealed file is not easily identified or found, additional time may be needed to research and locate the file.

Remarks: Processing of orders for pre-adoption birth records will normally take from four to six weeks.


Adoption

When a U.S. court approves an adoption for a child born in Oregon, the Oregon Vital Records office prepares a new birth record and the original birth record and the adoption documents are placed in a sealed file.

Vital Record sealed files can only be opened through a court order from an Oregon Court even when the birth parents are known to the adoptee or the adoptive parents.

Remarks: Under Oregon law when adoptees reach the age of 21 they may order their pre-adoption birth record.

Oregon law provides for the issuance of a Certificate of Foreign Birth if a child born outside the United States to parents who are not U.S. citizens is adopted through an Oregon Court.

A child born in Canada to parents who are not U.S. citizens will have a new birth certificate created by the Province where the birth occurred.

If a Certificate of Foreign Birth has already been filed in another state and the child is re-adopted by a second set of adoptive parents, the documents should be forwarded to the state where the first Certificate of Foreign Birth record was filed.

Paternity and Parentage

New Voluntary Acknowledgment of Paternity forms are now available in English and Spanish for use by hospitals and parents.

The new forms have a different format but do not include legal changes, so older forms that are dated 01/08 will still be accepted.

Remarks: For earlier death records, write to the Probate Court in the county where the event occurred.

Order Certificate of Stillbirth

Certificates of Stillbirth are available to the biological parents when a fetal death record is on file for a stillbirth that occurred after January 1, 1999. This is a memento certificate, not a legal record.

As of February 3, 2010, the fee for this certified memento is $20. You may also order a certified copy of the fetal death certificate for $20 if it is needed for legal purposes.

Remarks: Currently Oregon law requires the filing of a fetal death certificate when fetal death occurs after the fetus reaches 350 grams or 20 completed weeks of gestation.
