Obtaining certified copies of vital records through the Center for Health Statistics can be made with the following steps;
Mail-In - Takes 3 to 4 weeks to receive a certified copy of a certificate;
Regular Fee: $20.00
Express: additional $18.30.

Notes: Funeral directors collect information about
the decedent from an informant (usually a family member or close
personal friend of the decedent). Cause-of-death information is
generally provided by a certifying physician, medical examiner, or
coroner.
Notes: The Washington Death with Dignity Act,
Initiative 1000, codified as RCW 70.245, passed on November 4, 2008
and went into effect on March 5, 2009. This act allows terminally
ill adults seeking to end their life to request lethal doses of
medication from medical and osteopathic physicians.
Remarks: Under the Death with Dignity Act, the
Department of Health collects information from healthcare
providers, reviews this information for compliance with reporting
requirements, contacts the healthcare provider if the information
is incomplete or inadequate, and produces an annual statistical
report.
Notes: Reporting of fetal deaths to the state
is required only when the gestational period is twenty weeks or
more. Similar information is collected for fetal deaths to mothers
who live in Washington State, but deliver in another state or
country.
Remarks: Fetal deaths are sometimes referred to
as stillbirths or miscarriages. Fetal Death Certificate System
contains records on all fetal deaths occurring in the state and
nearly all fetal deaths to residents of the state.
Notes: Infant deaths are defined as deaths of
children under one year of age. Information from the Washington
State Birth and Death Certificate Systems is combined to produce
records of all infant deaths occurring in the state and nearly all
infant deaths to residents of the state.
Remarks: Information is gathered about the
mother, the father, the pregnancy, the child, and the
cause-of-death.
