Amendments and Corrections
A certificate on file in this office may be amended only in accordance with Kentucky statutes and administrative regulations.
Remarks: If you need to correct information on a certificate, please contact the Vital Statistics office between 8 a.m. and 4:30 p.m. (EST), Monday-Friday.
An apostille/certificate of authentication is a document intended for international use and is obtained only through the Kentucky Secretary of State's Office.
Remarks: A certified copy of a vital record
may be used for this purpose and may be obtained during the
Secretary of State's process.
How to order a certified copy:
By Mail - Mail a check or money order (no cash) payable to the Kentucky State Treasurer along with the necessary information or documents;
UPS - For faster delivery, request delivery by UPS when placing orders for vital records, an additional fee is charged for this service.;
In-Person - Stop in our office at 275 E. Main St. in Frankfort to complete an application for a certified copy of a birth, death, marriage or divorce certificate between the hours of 8 a.m. - 3:30 p.m. EST, Monday-Friday.;
VitalCheck - Secured online applications to request for vital records, which accepts all major credit card payments, can be made through VitalCheck with an additional charge for their service.

Consular Report of Birth Abroad
A document issued by an American embassy or consulate
reflecting the facts of a birth abroad of a child acquiring U.S.
citizenship at birth through one or both parents. This record, also
known as an FS-240, along with the Certification of Birth, DS-1350,
are acceptable as proof of birth and U.S. citizenship for all legal
purposes.

Report of the Death of an American Citizen
A document issued by an American embassy or consulate
reflecting the facts of a death abroad of an American citizen. The
document is based upon the local death certificate.
Stillbirth Certificate
Illinois law created a process for establishing a parent-child relationship between a child and his or her intended parents when the child is being carried by a gestational surrogate.
Remarks: The fee for a certified copy of a death certificate is $6.00. Additional copies are $6.00 each.

Certificate of Witness to Marriage
A document issued by an American embassy or consulate
reflecting the facts of a marriage abroad in which at least one
party is an American Citizen. A consular officer must have attended
the ceremony. This accommodation was discontinued in
1987.
