Vital Records no longer maintained by the Office of Vital
Records and Statistics are found at the Division of Archives &
Records Service. This includes 1) death records 50 years old or
more; 2) birth records 100 years old or more; and 3) marriage and
divorce records 75 years old or more.
Remarks: All requests must include a copy
of State-issued or Government-issued Photo ID of the person
requesting the Certificate. Expired cards or documents will not be
accepted.
Mail-In - Takes 7 to 10 business days to receive a certified copy of a certificate. Mail requests are usually processed and mailed within 3 weeks.
Phone Service - Place an order over the phone using your credit card. Call toll-free 866-632-2602 during regular business hours.
Walk-In - Apply in person at our Salt Lake City office; Monday - Thursday, 7:00 a.m. - 6:00 p.m. Requests taken after 5:30 p.m. will be processed the next business day.
VitalCheck - Secured online applications to
request for vital records, which accepts all major credit card
payments, can be made through VitalCheck with an additional charge
for their service.

Notes: The Lieutenant Governor is responsible
for affixing the Great Seal to certain public documents or
documents signed by Utah Notaries Public.
Remarks: It is important to specify which
foreign country is involved, so this Office can determine if an
Apostille or "regular" Authentication is necessary. Use of the
wrong certificate can cause a document to be rejected by the
foreign country.
