Requests for copies of vital records may be made for aggregate or identifiable data which is obtained by combining like data in a manner that excludes the identification of the individual described in the data.
Remarks: The individual or organization requesting the vital records data must show that the data will be used for medical or scientific research, or that the release of the data is authorized by the Freedom of Information Act.
Amendment/Correction on a Certificate
It is advised that you call the local registrar of the town where the vital event occurred for further information, and to know what type of documentation you will need to provide.
Remarks: The process to make changes to a certified copy vital records varies depending on the type of legal change needed and how recently the record was filed.
Town Vital Record Contact Information
The vital records registrar in each 169 town is responsible for maintaining a registry of all births, marriages, civil unions, deaths and fetal deaths that occur within its town.

State Vital Records Office
The State Vital Records Office at the Connecticut Department of Public Health maintains a statewide registry of all births, marriages, civil unions, deaths and fetal deaths which have occurred in CT since July 1, 1897.
Remarks: For information on vital records prior to July 1, 1897, you must contact the town where the vital event occurred.
The State Library's Barbour Collection includes most Connecticut vital records (births, marriages, and deaths) to about 1850.
Remarks: Most vital records to about 1900 have been microfilmed by the Genealogical Society of Utah and are available for use at the State Library or through LDS Family History Centers.

To be eligible to receive a copy of a birth certificate, you must be able to document that you are related to the registrant

Notes: Requests sent to the State vital Records Office require a postal money order made payable to the Treasurer, State of Connecticut.
For a person born in the State of Connecticut
The birth certificate that you will receive will list the child's adoptive name and the names and demographic information pertaining to the adoptive parents.
Fee: $30.00
Remarks: If the adopted person was born in another state, you must request the birth certificate from the state in which the person was born.
Child Adopted from a Foreign Country
Adoptive parent(s) may purchase a Certification of Birth for, (1) a child born out of the country and had the adoption finalized in Connecticut, or (2) had the foreign adoption finalized out of the country and authenticated/validated by a Connecticut court.

Certificate from an Adoption Sealed File
If a new birth certificate is created at the time a person is adopted, the original birth certificate becomes confidential.
Fee: $30.00
Remarks: In order to gain access to the original confidential birth record, a person must receive a written order from a court of competent jurisdiction allowing such person access to the original birth record.
If a mother is not married at the time a child is born and has not been married at any time between conception and the birth of the child, no father will be named on the birth certificate unless both parents complete an Acknowledgement of Paternity form, or unless ordered by a court of competent jurisdiction.
Fee: $50.00
Notes: Parents may choose to complete the Acknowledgment of Paternity at the hospital when the child is born or may do it later.
Court Ordered Paternity
Court ordered paternity becomes necessary when the parents disagree about the identity of the father, or if either parent is unwilling to sign the Acknowledgement of Paternity form.
Remarks: If paternity is established through a court order, a certified copy of the court order must be submitted to the Department of Public Health and the father's name will be included on the child's birth certificate.

An Apostille is a special form of certificate authentication prepared under the terms of an international treaty known as "The Hague Convention Abolishing the Requirement of Legalization for Foreign Public Documents."

Notes: In Connecticut, the Office of the Secretary of State is designated to issue certifications by Apostille.
Remarks: The Apostille simplifies the authentication process by exempting documents certified with the Apostille at the state level from any additional certification by a higher authority, such as at the US Department of State or consulate.

Complete the Application for copy of Death Certificate available on this site and mail it or bring it in person to the vital records office of the town or city where the decedent resided at the time of death, or to the State Vital Records Office.

Time-frame: From 1897 - present
Notes: Connecticut death certificates are available to the general public. Anyone over 18 years of age may request a copy of any Connecticut death certificate.
Remarks: You must provide basic information such as the name of the decedent, the approximate date of death, and the town where the death occurred, so that the proper death record can be identified.
The Certificate of Stillbirth may be requested for a fetus that reached at least twenty weeks gestation and died before birth. Only the parent of the stillborn may request the Certificate of Stillbirth.
Fees: $20.00 each certificate
Notes: A Certificate of Stillbirth is a document issued by the State of Connecticut Department of Public Health.
Remarks: The document cannot be used to prove identity, or any other legal purpose.

Requests for certified copies of a marriage or civil union certificate may be submitted to the city/town where the marriage or civil union ceremony took place, to the town in which either of the parties resided at the time the marriage or civil union took place, or to the State Vital Records Office.

Notes: State Office only has short form certified copies are available. Additional copies of the same record ordered at the same time are $8.00 each.

Marriage License and Certificate
If you are planning to marry, you and your spouse-to-be must appear in person at a local vital records office to apply for a marriage license. The marriage license is issued to ensure that you and your spouse-to-be are eligible to be married.

Notes: You must apply for your marriage license at the vital records office of the town where the marriage is to take place.
Remarks: As of November 12, 2008, same sex marriage became legal in Connecticut. The eligibility requirements and application process for same sex marriage are identical to the requirements for opposite sex marriage.
Civil Unions License & Certificate
A civil union is a legal union between two people of the same sex which grants the parties all the same benefits, protections and responsibilities under state law, whether derived from the general statutes, administrative regulations or court rules, policy, common law or any other source of civil law, as are granted to spouses in a marriage.
Licensing Fee: $30.00 each certificate
Certification Fee: $20.00 each certificate
Notes: Persons wishing to enter into a civil union may do so prior to October 1, 2010.
Remarks: As of October 1, 2010, Connecticut's civil union laws will be repealed and no civil union license may be issued or filed.

To obtain a copy of your Divorce Decree or Dissolution of Civil Union Decree, you must request it from the Superior Court where the divorce or dissolution was granted. Please call the appropriate court for fees and requirements.
